- Write down the tasks and times your family cannot cover. Confirm clinical questions with the discharge or care team.
- Ask an agency about its services, staffing, minimum visits, backup arrangements and written charges.
- Compare the same schedule across options. Record what remains unconfirmed.

- ACL Eldercare Locator — Connect with local aging-services resources and learn which organizations serve your area. Ask what information, assistance or programs they can provide.
- Medicare home-health information — Use Medicare’s coverage information to prepare questions. Ask the care team and relevant insurer to confirm eligibility, covered services and costs for your situation.
