Organization only
Family Elder Planning provides educational and organizational checklists for families. It is not medical, nursing, legal, financial, insurance, tax, or emergency advice.
Use licensed professionals for decisions
Use these pages to prepare better questions for licensed professionals, including doctors, nurses, pharmacists, therapists, home-health providers, elder-law attorneys, financial professionals, insurers, or emergency services when appropriate.
Emergencies
If there is an urgent symptom, immediate safety concern, suspected emergency, severe change, fall, medication emergency, breathing problem, chest pain, severe confusion, or any other urgent concern, contact emergency services or a licensed clinician. Do not rely on this site to decide what is urgent.
Digital products
The First 14 Days Home Kit is a protected digital browser packet. It opens as fillable pages that can be printed or saved as PDF; entries are not stored for a later visit, and nothing is shipped. It is intended for personal family organization and may not be resold, republished, or presented as professional advice.
The Family Care Coordination System is a local-first organizational workspace. Care-plan information is stored in this browser on this device unless you deliberately export a Family File or print/save a Share Pack. Local autosave is not a backup; shared browser profiles and cleared site data create loss/exposure risks. You are responsible for safeguarding exported or printed copies and verifying current information with the appropriate people and professionals.
Sponsored access
An approved organization may sponsor access through bounded co-branding. Sponsorship does not mean Family Elder Planning evaluated, ranked, recommends, certifies, or endorses that organization or its services. A sponsor does not receive the local family plan through the product. Sponsor contact links lead to a separate service with its own privacy practices, and you choose what to discuss or share.
The Family Care System does not replace an agency’s scheduling, CRM, EMR, EVV, billing, clinical record, emergency process, or professional services. Business pilot terms require a separate legally reviewed agreement.
No guarantee of outcome
Every discharge situation is different. Family Elder Planning cannot guarantee that a checklist will include every task, risk, question, benefit, or responsibility that applies to your family.
