Elderly parent emergency binder checklist
After discharge, keep the most important information in one place so family can act quickly without digging through messages and papers.
Include
- Current medication list and pharmacy contact
- Doctor names, specialties, phone numbers, and portals
- Hospital discharge instructions and follow-up appointments
- Insurance, Medicare/Medicaid, ID cards, and emergency contacts
- Home health, caregiver, facility, and family task-owner contacts
- Copies or notes about key legal/financial documents if the family already has them
Keep it usable
- Use one folder or binder.
- Put the newest medication list on top.
- Remove outdated instructions or clearly mark them old.
- Tell family where the binder is.
