Every discharge is different. Before your parent leaves, ask the care team to explain what is expected, what deserves a call, and what is urgent.
Ask the care team to define
- Expected recovery symptoms
- Symptoms that should trigger a same-day phone call
- Symptoms that should trigger urgent care or emergency care
- Who to call after hours
Examples to clarify
- Fever, worsening pain, breathing trouble, chest pain, sudden weakness, severe confusion
- Fall, near-fall, new dizziness, inability to walk safely
- Not eating or drinking, vomiting, diarrhea, dehydration concerns
- Medication error, severe side effect, wound changes, bleeding, swelling
