    “Hi, my parent was discharged on [date], and I am helping organize the first two weeks at home. I want to confirm the instructions, follow-up appointments, equipment, home health, and the best number to call if something does not match the paperwork.”

  2. Pharmacy

    “Hi, my parent was just discharged from the hospital, and I am helping organize their medication changes. Can you confirm which prescriptions are ready, which are delayed, and whether anything needs clarification from the doctor?”
